Please plan on arriving about 15 minutes early for your first visit. You will be required to fill out patient registration, patient history, and review of informational forms. This information will help your doctor to evaluate your current health as it relates to your eye care. We update this information annually to ensure we have accurate records.

Please bring your insurance identification cards and any medications you are taking. If you have been under the care of another doctor, we may ask your permission to obtain a copy or transfer some of your records.

Cancellations of Appointments: Please call The Eye Center promptly if you are unable to attend an appointment. This time will be reallocated to another patient.
